Authorization for Release of Information of Physician / Therapist 

Since you are currently in counseling, Big Brothers Big Sisters Kansas City is required to reach out to your therapist for a reference. Please complete this release form so that your therapist can provide a reference. 
I authorize the release of information to Big Brothers Big Sisters about the nature of my treatment and an assessment of my ability to provide directed friendship to a child. The purpose of this information is so that Big Brothers Big Sisters can assess my strengths and weaknesses in preparation for a possible match with a child. This consent expires one year from the date below unless expressly revoked earlier.
