- Voluntary Participation: Big Futures is a voluntary program that provides mentoring, coaching, goal-setting, resource navigation, and related support through Big Brothers Big Sisters staff. Participation does not guarantee employment, education, housing, financial assistance, or any other specific outcome.
- Participation & Conduct: I agree to provide accurate information, communicate with Big Futures staff, participate in developing and working toward my goals, maintain respectful and appropriate relationships with staff and other participants, and follow program policies and expectations.
- Confidentiality & Safety: Information I share with Big Futures will be treated with appropriate confidentiality. I understand that information may be shared when necessary or required by law, including circumstances involving safety concerns, emergencies, or other legal obligations.
- Program Services & Referrals: I understand that Big Futures staff may provide mentoring, guidance, and connections to community resources. Big Futures does not guarantee services provided by outside organizations, and participation does not create an entitlement to financial, legal, medical, mental health, employment, or other professional services.
- Participation and Program Closure: I may choose to discontinue my participation in Big Futures. I understand that Big Brothers Big Sisters may also discontinue services when appropriate, including when there is a significant safety concern, repeated inability to maintain contact, serious violation of program expectations, or when my needs are outside the scope of the program.
I certify that the information provided in this application is accurate to the best of my knowledge. I understand that I may ask questions about Big Futures and its policies before or during my participation.
