Thank you for your interest in serving as a mentor with the Massachusetts Bay Community College Mentor Program through Career Services, in partnership with Big Brothers Big Sisters. 


This enrollment form is designed to help the program team understand your professional background, education, areas of expertise, availability, mentoring interests, and match preferences.

Mentors provide trusted guidance and career perspective to MassBay students through one to one mentoring. 


The relationship may include career exploration, resume or interview support, networking, job shadowing, education planning, workplace readiness, and encouragement as students move toward internships, employment, transfer, or other goals.


Please answer as honestly and completely as you can. Questions marked optional may be skipped. Information shared in this form should be used for program administration, mentor screening where applicable, matching, and student support.

Agency Weapons Policy

A Big who owns firearms or weapons and stores them at their residence or any other location will truthfully disclose this information to Big Brothers Big Sisters. A firearm is defined as a weapon from which a shot it discharged by gunpowder. The Big must attest to the fact that any weapons, firearms, or ammunition ownership are licensed, permitted, registered, and handled in accordance with al the state and federal laws. The Big will lock firearms and weapons in a secure location whenever a Little is at or near the storage area. Ammunition will also be stored separately and be inaccessible to the Little. The Big will not carry a firearm while with their Little, regardless of whether the volunteer has a permit to carry a firearm. This includes no firearms in vehicles. 


Child safety is our priority. We request and require all applicants to submit this information to run national and state background checks. No dashes.
Release of Information Authorization for Background Check
By my signature below, I hereby consent to the preparation by Sterling Volunteers, a consumer reporting agency located at 4511 Rockside Road, 4th Floor, Independence, OH 44131, 855-326-1820 Option 3, www.sterlingvolunteers.com, of background reports regarding me and the release of such reports to Big Brothers Big Sisters and its designated representatives, to assist BBBS in making an employment and/or volunteer/non-employee position decision involving me at any time after receipt of this authorization and throughout my employment and/or volunteer/non-employee position, to the extent permitted by law. To this end, I hereby authorize, without reservation, any state or federal law enforcement agency or court, educational institution, motor vehicle record agency, credit bureau or other information service bureau or data repository, or employer to furnish any and all information regarding me to Sterling Volunteers and/or BBBS itself, and authorize Sterling Volunteers to provide such information to BBBS. 


Background checks are renewed at a minimum of every three years.  
Release of Information Authorization of Driving Record from Massachusetts Department of Transportation Registry of Motor Vehicles
I authorize Big Brothers Big Sisters ("BBBS" or the "Agency") to request my driving record from the Massachusetts Department of Transportation Registry of Motor Vehicles.

I also give consent for the Agency to release my driving record to other interested parties, including insurance companies or agencies that provide insurance coverage to the Agency.

I understand that the Agency will use these records to evaluate my suitability for any driving duties related to my current position or the position for which I am applying.

Additionally, I consent to the review, evaluation, and use of any driving records or other motor vehicle records provided by me or others to the Agency.

This written consent satisfies all applicable Federal, State, or local laws regarding the release of driving records.
Photo Consent Release 

I irrevocably grant Big Brothers Big Sisters, MassBay, their affiliates, successors, partners, sponsors, donors, and authorized third parties the right to use my name, image, likeness, biographical information, recordings, writings, and artwork (the "Released Material"), in original or modified form, for advertising, publicity, marketing, fundraising, and related purposes, in any media now existing or later created, worldwide, in perpetuity, at their sole discretion — without further consent, compensation, or royalty to me.

I assign to BBBS and MassBay all rights, title, interest, and copyright in the Released Materials, including the right to register and enforce those copyrights. I understand these materials become BBBS's property, that only BBBS and MassBay may authorize third-party use, and that I have no right to review or approve them before use.

I acknowldge BBBS and MassBay has no obligation to use the Released Materials, and no liability to me for any editing, alteration, or distortion resulting from their use.

I release BBBS and MassBay, its affiliates, employees, agents, and partner companies from all claims, demands, and liabilities — including compensation, defamation, invasion of privacy, or other rights violations — arising from the exercise of these rights or the appearance of the Released Materials in any publication or production.


Disclosure of Non-Discriminatory Policy
I understand that Big Brothers Big Sisters and MassBay is committed to the inclusion of all people, supporting all identities.

Support Specialists may try to accommodate matches based on similar affinities. However, except for matching like gender identities, which is currently a requirement, all may be matched with individuals who identify as members of any race, ethnicity, immigration status, sexual orientation, ability, or religious preference.

If you feel this policy has not been followed or wish to appeal a decision related to it, you may contact our Chief Executive Officer or our Board Chair to request a review.

I certify that all information I have provided or will provide in this application is true, accurate, and complete to the best of my knowledge. I have answered and will answer all questions to the best of my ability and have not withheld any information that could affect my volunteer application. I am aware that Big Brothers Big Sisters will verify the information provided. Any misrepresentations or omissions may lead to immediate rejection or termination as a volunteer.

During my involvement with the Big Brothers Big Sisters and MassBay program, I agree to promptly inform my contact person about any infractions, violations, charges, or convictions related to civil, domestic, or criminal matters. I agree to update the agency about any changes to the information provided during the enrollment process. I agree to complete questionnaires to evaluate and improve program services throughout my time in the program. I commit to timely communication and follow-up with all agency staff as required.

I fully understand and consent to these conditions as part of my application process for volunteering with Big Brothers Big Sisters and MassBay.
