Release of Information Authorization for Background Check

The information I provided in this application may be used to conduct a background check, to include a search of public domain records, driving records check, military records, criminal history check, fingerprint search, and other records where required by local, state, or federal law for volunteers working with youth. I understand that BBBSMV incorporates, and complies with, DOJ OJP Determination of Suitability to Interact with Participating Minors Special Condition requirements as referenced here (https://www.ojp.gov/funding/explore/interact-minors).

At the time of the interview, I will be asked to complete the Volunteer Applicant Information and Background Consent form, which will include my social security number, to complete a layered criminal history check.

I will be required to provide a valid driver's license and proof of auto insurance at the enrollment interview.

As part of the BBBSMV enrollment process, I will be asked to provide additional personal information, including  completion of an in-person interview, prior to making any recommendations for a match.

The information I have provided to BBBSMV will be kept confidential, unless disclosure is required by law. However, I understand that certain relevant information about me will be discussed with the parent/guardian of a child who is a potential match (this might include demographic information, information relevant to parent/child preferences, and any information relevant to a child's safety or well-being).
