This form is to be completed by a school counselor or teacher. Due to the importance of the follow-through, commitment and leadership needed in a role model to younger students, we feel confident that a school counselor or teacher will understand the important position this student will have in our program. This student will be a representative of the High School they attend and will be mentoring an elementary school student.
