Thank you for your interest in Big Brothers Big Sisters of Southern Minnesota. The first step in the process is to complete this application. Please complete an application for each child you would like to enroll in the program. Volunteer (Bigs) and youth (Littles) are matched based on compatibility and shared interests. We want to make sure that we find the best Big for your child.

Parent/Guardian Agreement

I give permission for my child to participate in the Big Brothers Big Sisters program.  I understand that the BBBS agency reserves the right to accept or deny any volunteer or child, or close a match at any time.  BBBS is not obligated to match my child with a volunteer. As a part of the enrollment process, I will be asked to provide additional information. I give permission for my child to communicate electronically/virtually with their Big and BBBS staff, and complete a Child Safety & Sexual Abuse Prevention Training with BBBS staff. If my child is matched with a Big Brother or Sister I agree to support my child’s match and to immediately report any concerns I might have to the BBBS staff.

 

Medical Release

I, as the parent or legal guardian of the above mentioned minor child, hereby grant authority and permission to Big Brothers Big Sisters of Southern Minnesota and my child's mentor to give permission to a medical care provider to allow for medical care and treatment in emergency situations where the medical care provider is unable to locate and contact the child’s parent or legal guardian. 

Confidentiality Policy

Big Brothers Big Sisters of Southern Minnesota respects the confidentiality of client and volunteer records and, with the exception of situations listed below, shares information about clients and volunteers only among the agency professional staff.

 

All records are considered the property of the agency and not the agency workers or clients or volunteers themselves. Records are not available for review by the clients or volunteers.

 

  1. Information will be released to other individuals or non-BBBS organizations only with the client or volunteer's written consent.
    2. Identifying information regarding clients and volunteers may be used in agency      
        publications, promotional materials or BBBS social media (ie. Facebook, website, 
        Twitter) unless the clients or volunteer request otherwise.

  1. For purposes of program evaluation, audit, or accreditation, and with the prior approval of the Board of Directors, certain outside bodies such as Big Brothers Big Sisters of America may have access to client and volunteer records. 
  1. Members of the Board of Directors or evaluators appointed by the Board have access to client files upon authorization of the Board of Directors. 
  1. Information shall only be provided to law enforcement officials or the courts pursuant to a valid and enforceable subpoena.  
  1. Information shall be provided to an agency's legal counsel in the event of litigation or potential litigation involving the agency. 
  1. At the time a child or volunteer is considered as a match candidate, information is shared between the prospective match parties. The information about the volunteer may include such items as: age, sex, race, religion, interests, hobbies, marriage, family status, sexual orientation, living situation, etc. Information about the child may include such items as age, sex, race, religion, interests, hobbies, family situation, etc. 

Publicity Consent

I, as the Legal Parent or Guardian of the Minor Child, irrevocably consent for myself and Minor Child to any and all uses and displays of my or Minor Child’s name, image, likeness, appearance, and basic biographical information in, on, or in connection with any advertising, publicity, marketing, or printed or electronic media, of any type, throughout the world at any time by Big Brothers Big Sisters of America, BBBS of Southern Minnesota, and any of their affiliates, successors, partners, sponsors, donors, and any entities or persons with whom they conduct any public relations, marketing, or fund raising of any type without further consent from me or Minor Child, without any royalty, payment, or other compensation to me or Minor Child, and with the release and waiver of any claims, actions, damages, losses, costs, expenses and liability of any kind arising from any such use.
