School Based Youth Application

Program Overview Letter

Dear Parent or Guardian, 

Your child has been selected to participate in the Big Brothers Big Sisters School Based mentoring program! This program allows volunteer mentors (adults or high school students) to provide local students with a positive, consistent role model and friend. These students are recommended by their teachers/counselors, and must be in grades K-8. To keep you informed, we want to provide you with a brief summary of the program. 

  • Your child will be meeting with their mentor 2-4 times per month ONLY on school property. 
  • Your child will be matched with a mentor with similar interests as your child based off their application and interview, as well as the volunteer mentor's application and interview. 
  • Together, your child and their mentor will discuss homework, participate in activities, play games, read books, and develop a friendship. 
  • You will be sent information 2 times during the school year by your child's assigned BBBS Match Support Specialist to give feedback about how your child is doing at school and with their mentor. 
  • ALL volunteers have been trained and screen through various steps including  application, interview, and references.

Parent/Guardian Agreement

My child has been chosen to participate in the Big Brothers Big Sisters program. My child will be meeting for one hour each week in the BBBS Mentoring Program ONLY on school grounds. Together they will discuss class assignments, participate in school activities, and develop a friendship.

 

I hereby make formal application to Big Brothers Big Sisters of Greater Birmingham, Inc., a non-profit Alabama corporation, to make available the services of the agency to my child, and, if possible, to assign to him/her a competent volunteer.  I recognize that the agency will use its best efforts to locate a responsible mentor to serve as a special friend to my child.  I agree to follow the rules and regulations set forth by Big Brothers Big Sisters.   I agree to hold Big Brothers Big Sisters free from any responsibility or liability. I further authorize the agency to release information in the child information section of the application. In addition, I will allow my child to attend agency activities/events and will hold Big Brothers Big Sisters free from liability.

  • To allow my child to share activities with a mentor ONLY on school grounds one hour a week.

  • To have my child complete a questionnaire containing questions about school, home life, and personal interests.

  • To fill out the forms necessary for Big Brothers Big Sisters to evaluate the program and improve it for future participants.

Social Media Consent

Publicity and promotional materials for Big Brothers Big Sisters are conducted in a variety of forms.  By selecting below I authorize Big Brothers Big Sisters and my child's Big to identify my child in the media as a Little Brother or Little Sister using their first name.  I also give permission for the use of other forms of media including agency newsletter, brochures, website, television, social media, and news outlet.

The following questions are intended to help us make the best match for your child. You may have a preference about the volunteer who may be matched with your child, and we will follow your preferences to the best of our knowledge and ability. BBBS does not discriminate, and volunteers with various characteristics may be accepted.
