  4. The BBBS agency is not obligated to match me with a youth and may deny my application or close my match at any time, and to protect all participants’ confidentiality, BBBS is not required to disclose reasons for doing so;

  6. As part of the enrollment processes, I will be required to provide additional personal information, including completion of an in-person interview;

  8. I understand that detailed accounts of child abuse or neglect, past or present, will be reported to proper authorities;
  9. I understand that certain relevant information about me will be discussed with the parent/guardian of a child who is a prospective match (this might include demographic information, information relevant to parent/child preferences, and any information relevant to a child’s safety or well-being);  
  10. It is my responsibility to update the agency if any of the information I provide on this application, in my interview, or any other information provided during the enrollment process changes (i.e. address, phone number, auto-insurance, new criminal charges, etc.)
  11. I agree to complete questionnaires throughout my time in the program to evaluate and improve program services;
  12. I agree to timely communication and follow-up with all agency staff as required by the agency.

Release of Information Authorization for Background Check

At Big Brothers Big Sisters of Flagstaff, we hold child safety in the highest regard. Because of this all of our volunteers and staff must consent to a layered background check. 
The information I provided may be used to conduct a background check, to include a search of public domain records, driving records check, adult criminal history check, and other records where required by local, state, or federal law for volunteers working with youth.  

I understand that my fingerprints will be used to check the criminal history records of the FBI. I understand that if I have criminal history record, BBBSF will provide the opportunity to complete or challenge the accuracy of the information in the record. I will be afforded a reasonable amount of time to correct or complete the record (or decline to do so) before BBSF can deny me for any volunteer opportunity based on information in the criminal history record. BBSF Program Director determines a reasonable timeframe for a volunteer to correct or complete their record as 20 days prior to receiving results

I understand the procedures for obtaining a change, correction, or updating of my FBI criminal record are set forth in Title 28, Cod of federal Regulations (CFR), Section 16.34. information on how to review and challenge my FBI criminal history record can be found at www.fbi.gov under "services" and then "Identity History Summary Checks" or by calling (304) 625-5590.

I understand that to obtain a copy of my Arizona criminal history in order to review/update/correct the record, I can contact the Arizona Department of Public Safety Criminal History Records Unit at (602) 223-2222 to obtain a fingerprint card and a Review and Challenge packet. Information on the review and challenge process can be found on the DPS website (www.azdps.gov).

Confidentiality and Digital Platform Policies

Big Brothers Big Sisters of Flagstaff respects the confidentiality of client and volunteer records and, with the exception of situations listed below, shares information about clients and volunteers ONLY among the agency professional staff. 


All records are considered the property of the agency and not the agency workers or clients or volunteers themselves. Records are not available for review by the clients or volunteers.


1. Information will not be released to other individuals of non-BBBS organizations without the client’ or volunteer’s specific written consent.

2. Identifying information regarding clients and volunteers may be used in agency publications or promotional materials unless the clients or volunteer request otherwise

3.For purposes of program evaluation, audit, or accreditation, and with the prior approval of the Board of Directors, certain outside bodies such as Big Brothers Big Sisters of America may have access to client and volunteer records. 

4. Members of the Board of Directors or evaluators appointed by the Board have access to client and volunteer files upon authorization of the Board of Directors. 

5. Information shall only be provided to law enforcement officials or the courts pursuant to a valid and enforceable subpoena. 

6. Information from client and volunteer files shall be disclosed pursuant to a valid and enforceable subpoena, court order, or other lawful order. 

7. Information from clients and volunteers files may be disclosed pursuant to state law that mandates that suspected child abuse and other crimes and misconduct be reported to the appropriate authorities, including law enforcement and AZDES Department of Child Safety. 

8. If an agency worker receives information indicating that a client or volunteer may be dangerous to himself or herself or to others, necessary steps may be taken to protect the appropriate party. This may include disclosure of information from client or volunteer files, as well as a medical referral or report to local law enforcement authorities. 

9. At the time a child or volunteer is considered as a match candidate, information is shared between the prospective match parties. The information shared about the volunteer may include such items as: age, sex, race, religion, interests, hobbies, marriage, family status, sexual orientation, living situation, etc. Information shared about the child may include such items as age, sex, race, religion, interests, hobbies, family situation etc. 


I irrevocably consent to any and all uses and displays of my name, image, likeness, appearance, basic biographical information, audio/video recordings, writings, artwork, and the like, in original form or in modified form, in whole or in part, in, on, or in connection with merchandise, advertising, publicity, marketing, fundraising, and the like, in printed or electronic media, of any type, throughout the world at any time by Big Brothers Big Sisters of America and Big Brothers Big Sisters of Flagstaff in their sole discretion, and by any of their affiliates, successors, partners, sponsors, donors, any entities or persons with whom they conduct any public relations, marketing, or fund raising of any type, and any other authorized third parties, without further consent from me, without any royalty, payment, or other compensation to me, and with the release and waiver of any claims, actions, damages, losses, costs, expenses and liability of any kind arising from any such use (the “Released Material”). 


In consideration of the mutual promises made herein, and for other good and valuable consideration, the receipt and sufficiency of which I hereby acknowledge, I hereby grant to BBBSA and its affiliates the right to use the Released Material as BBBSA and/or its affiliates may desire, in all media now existing or hereafter created and in all variations and forms including, but not limited to, internal or external publications or productions, informational or recruitment materials, marketing materials, fundraising materials, televised photography and/or recordings, advertisements, Public Service Announcements, and/or online and social media sites. The use of this information shall be at the sole discretion of BBBSA and/or its affiliates. I further grant to BBBSA and its affiliates the absolute right to use the Released Material in whole or in part, alone or in conjunction with any other image, name, writings or reproduction, in color or otherwise, for art, advertising, business, trade, or any other lawful purpose whatsoever, in perpetuity throughout the world. I understand and agree that all materials created by BBBSA and/or its affiliates that use the Released Materials are the property of and are owned by BBBSA, and that I cannot authorize their use by any other party. 


I further understand that BBBSA may authorize their use by a third party. I hereby irrevocably transfer and assign to BBBSA my entire right, title and interest, if any, in and to the Released Materials and all copyrights in the Released Materials arising in any jurisdiction throughout the world, including the right to register and sue to enforce such copyrights against infringers. 


I acknowledge and agree that I have no right to review or approve the Released Materials before they are used by BBBSA and/or its affiliates, and that BBBSA has no liability to me for any editing or alteration of the Released Materials or for any distortion or other effects resulting from BBBSA’s and/or its affiliates’ editing, alteration or use of the Released Materials. BBBSA has no obligation to use the Released Materials or to exercise any rights given by this Consent and Release Form.


I hereby release BBBSA and its affiliates, employees, and agents, as well as any partner companies, from all claims, demands or liabilities and related financial costs that I may now or hereafter have arising in connection with BBBSA’s exercise of the rights hereby granted, and/or with the appearance or the Released Materials in any publication or production. These include, without limitation, claims for compensation, defamation, or invasion of privacy, or other infringements or violations of personal or property rights of any sort whatsoever.

Social Networking Match Policy

Rationale: Big Brothers Big Sisters recognizes that online social networking (Facebook, Instagram, Twitter, LinkedIn, Blogs, etc.) has become a very popular communications tool and that many of our Big Brothers/Sisters and Little Brothers/Sisters use these websites. In general, personal use of social networking sites is typically for communications between peers, including friends and family. BBBS wants to reiterate that your relationship with your Little is that of a mentor in addition to a friend. Not all social networking sites are designed to support mentoring relationships.

Please proceed thoughtfully when inviting or accepting an invitation from your Little to a social networking site. If you have any doubts, please contact your Match Support Coordinator and/or postpone your invitation or acceptance. If you do choose to participate in a social networking site with your Little, you must adhere to the following guidelines regarding any postings related to your match or affiliation with BBBS:

• Do not participate in any online communication with/about your Little unless you have discussed this with the parent/guardian and Match Support Specialist and received approval. • Give the parent/guardian and BBBS your website address, and when possible, Link your page to the BBBS page on that site so that we can follow along with your postings. We are active on Facebook and Twitter. If you have Facebook, please join the BBBS of Flagstaff Big Connection Group after becoming matched.
• Never post any pictures of your Little online unless you have the written consent of the child’s parent/guardian. That consent form should be filed with BBBS.
• Never post the last name of your Little, his/her parent/guardian or BBBS staff. • Never post any contact information for your Little.
• Never post any identifying information about your Little (i.e.: child’s home address, school the child attends, where the parent works, etc.).
• Never post anything that could be perceived as judgmental or offensive to your Little or his/her parent/guardian.
• Make sure that your page settings are set to private, so that only people you approve can view your online content. Ensure that BBBS and the parent/guardian are approved if you plan to post content related to your match.
• Honor confidentiality guidelines within your match; be cautious when posting detailed information about conversations with your Little, their parent/guardian or with BBBS staff. Posting generalized information or status updates is typically appropriate, but lengthier postings or blogs containing in-depth information should be approved by the parent/guardian and by BBBS prior to posting.
• Be aware of other postings on your page that are not BBBS related or are inappropriate for minors. Know that as a BBBS volunteer you are a role model at all times. Please ensure that all contents are appropriate and kid-friendly, in case your Little or his/her family were to read your social networking page. If the social networking site allows, create a separate group for your Little, his/her parent/guardian and BBBS staff that limits your Little’s access to your general or universal postings.
• When in doubt about whether or not something is appropriate to post, consult with your Match Support Specialist prior to posting.

Overnight Visit Match Policy

An overnight visit may occur only when approval is given in advance by BBBS’ professional staff and the child’s parent/guardian in accordance with the terms of this policy. Failure to comply with this policy will result in immediate match suspension or closure.

Overnight activities should be reserved for special occasions where there is an opportunity to promote: youth education, life skills, leadership development, career exploration, and/or participation in outdoor sports/environmental activities or family vacations/trips. The following conditions must be met before an overnight visit will be considered for approval:

• The activity would enrich the child’s healthy development
• The match has been in effect for at least 1 year
• The Big is at least 18 years old
• The Big has demonstrated good judgment and reliability in his/her commitment to the Little and has adhered to child safety guidelines, program policies and procedures

All overnight visits must adhere to the following during the overnight visit:

• The Big is responsible for the supervision of the Little and for providing a safe and secure environment. The child should never be left unsupervised or in the care of someone else.
• Use of alcohol, drugs or tobacco is prohibited when with the child.
• Under no circumstances should a Little sleep together in the same bed or sleeping bag with the volunteer, or other or youth.
• Provisions for privacy for separate sleeping, changing of clothes and showering for both the volunteer and the Little need to be set out in advance.
• Match parties are expected to follow BBBS child safety policies and procedures at all times.

Approval for an overnight visit must be secured from the parent/guardian and the Match Support Specialist to be approved by the Program Director in advance to ensure that all conditions are met.

All overnight permission conversations and interviews will be documented.

Special approval must be obtained from the Program Director for any activities that include any of the following:

• An outing that includes more than 3 consecutive nights
• Travel out of the country

The agency may at its own discretion plan group events/activities which may include overnight accommodations for a group of matches.

Media Consent and Release

I irrevocably consent to any and all uses and displays of my name, image, likeness, appearance, basic biographical information, audio/video recordings, writings, artwork, and the like, in original form or in modified form, in whole or in part, in, on, or in connection with merchandise, advertising, publicity, marketing, fundraising, and the like, in printed or electronic media, of any type, throughout the world at any time by Big Brothers Big Sisters of America and [BBBSA affiliate] in their sole discretion, and by any of their affiliates, successors, partners, sponsors, donors, any entities or persons with whom they conduct any public relations, marketing, or fund raising of any type, and any other authorized third parties, without further consent from me, without any royalty, payment, or other compensation to me, and with the release and waiver of any claims, actions, damages, losses, costs, expenses and liability of any kind arising from any such use (the “Released Material”).

Application Acknowledgement

I understand that this is an application for a volunteer opportunity and is not a promise or commitment by Big Brothers Big Sisters.

I certify that all information I have provided or will provide to Big Brothers Big Sisters, including this application, is true, accurate, and complete to the best of my knowledge. I certify that I have and will answer all questions to the best of my ability and that I have not and will not withhold any information that would affect my application for a volunteer position. I understand that information contained in my application will be verified by Big Brothers Big Sisters. I understand that misrepresentations or omissions may be cause for my immediate rejection as an applicant or my termination as a volunteer.

At any time while involved with the Big Brothers Big Sisters program, I agree to immediately inform my Big Brothers Big Sisters contact person of any and all infractions, violations, charges, and convictions related to any civil, domestic, or criminal matters.  I understand that BBBS staff needs to be fully informed to provide the best guidance or support possible.
